New Patient Information
We’d like to proudly welcome you to University Dental Group; we hope you enjoy your stay with us as we fulfill you and your family’s needs. We look forward to providing care that is comfortable, comprehensive and affordable. As you learn more about our dental office, take a moment to read our new patient information below. You’ll find valuable information about financing your treatment, patients forms you’ll need to fill out, and what you can expect from your first visit.
Your First Visit
During your first visit, you’ll be met with a friendly greeting and given a chance to speak with our front desk team members. You’ll get the chance to fill out new patient paperwork and receive a new patient packet, complete with all the information you need to learn about our dental office. Afterward, we’ll determine the most ideal time and day for your visit with one of our dentists, which typically includes a checkup and cleaning with time permitting.
New Patient Forms
When you become a new patient, it’s incredibly important to learn as much medical information as possible from you. This allows our dental office to meet your needs to the best of our ability as well as confirm which treatments will best help you achieve optimal oral health. Once you’ve filled out these patient forms, simply hand them to our front desk and we’ll make sure to see you as soon as possible.
Dental Insurance Accepted
Dental insurance can make it much easier to pay for the dental services that you need, whether it’s for a new restoration or a basic checkup and cleaning. At our dental office, we accept dental plans from most major insurance providers. We are in-network with Delta Dental, Blue Cross Blue Shield, Altus, Aetna, and Metlife. Our team is experienced in helping patients navigate their insurance and more than happy to confirm their benefits before beginning treatment.
Financing Available
Even if you don’t have dental insurance or you need help paying for a treatment that your benefits didn’t completely cover, our dental office is proud to work with third-party financing companies to make payments more manageable. Programs like CareCredit are designed to stretch the total cost of your payment across multiple months, oftentimes with little to zero interest attached. The last thing you should do is put off treatment that you need, so give us a call or read more by following the link below!
